Individualized Plan of Care
We encourage our residents and their families to participate in establishing a plan of care that meets the social, spiritual, emotional and physical needs of our residents, their families and friends. We realize the importance of developing and implementing an individualized plan of care that meets the resident’s medical and social needs.
Result Oriented Plan of Care
Upon admission, our interdisciplinary team consisting of physicians, therapists, nurses, social services, dietary and other support staff, evaluates each resident to determine the course of treatment that will achieve optimum results. Our goal is to return each resident to the highest level of independence, whether returning home or to a lower level of care.
Social Services Support and Discharge Planning
Our Social Service Department assists families and residents with physician consults, psychiatric services, transportation and discharge planning. Discharge planning is geared towards meeting each individual resident's needs and goals, including their safety. Families are invited to participate in regular Patient Care Plan Conferences and Family Support Groups.
