Who we are
Horizon West HealthCare, Inc., is a privately held California corporation that provides administrative support and consulting services to 34 skilled nursing and assisted living and retirement communities throughout California and Utah. These individual communities provide a broad range of healthcare services including nursing care, rehabilitation therapy and other specialty services. We define "Our" as the communities that Horizon West HealthCare, Inc., contracts with individual Service Agreements.
