Individualized Plans of Care
Our residents' care plans include much more than 24-hour skilled nursing care. The plans of care also address the social, spiritual and emotional needs of residents, their families and friends. Our staff realizes the importance of care plans which are individualized and meet the resident's medical and social needs. These care plans are based on input from our Interdisciplinary Team comprised of physicians, therapists, nurses, social services, dietary, and support staff.
Upon admission, our rehabilitation and skilled nursing teams evaluate each resident to determine the course of treatment that will provide the optimum results. Our goal is to return each resident to the highest level of independence. Whether returning to their home, or to a lower level of care. For those who return home we offer full discharge planning and an evaluation to address safety issues and concerns.
For residents who need 24-hour care, our professional staff delivers short-term and long-term daily nursing services, as well as support programs for residents and their families.
