Individualized Plans of Care
Here at Heritage Care Center, we understand that each resident will have very individualized medical and social needs, upon admission, nursing and rehabilitation staff evaluates each resident to create a plan of care that is best suited to their individual needs. Our goal is to assist each resident to the highest level of independence, whether it be to return home or to a lower level of care. For those who return home, we offer full discharge planning and evaluation to address any safety concerns. We also offer extended skilled nursing care for those who need it.
