Horizon West HealthCare contracts with 27 Skilled Nursing Facilities, 4 Assisted Living Communities, and 3 Active Retirement Communities located throughout California and Utah. Selecting Care

Selecting a community that will provide care is merely the first step. The most important task at hand is to discover the level of care you may require.  Below is an abbreviated list that may help you assess your current or upcoming care needs.

• What type of care do I require?
• Do I want to maintain my independence while knowing help is near?
• Will I require 24-hour medical and nursing attention?
• Do I need assistance with routine daily activities?
• Do I need therapeutic and/or rehabilitative therapy services?
• Will I need to make a change in my level of care, either to higher level or lower level?
• Would I prefer to live in a community that offers amenities such as housekeeping, meals, transportation, etc.?

Active Retirement

It’s all the comforts of home, only better. Having the added security and knowledge that help is just moments away if it’s needed. The active retirement communities provide well-appointed amenities to cater to your comfort as well as your active lifestyle. With gracious dining areas and restaurant style meals often included in the cost of your fees, you may start to wonder why you didn’t move in sooner.
           
Peace of mind also includes the health and safety of the residents. Offering pull cord alert systems, as well as services such as housekeeping, assistance with transportation, and many scheduled social and recreational activities. These communities strive to enhance your active lifestyle. Assisted Living

Assisted Living communities offer living comforts such as private apartments or shared rooms.  These communities also offer the needed support for daily activities such as bathing, dressing, and supervision of medications.

The  Assisted Living communities offer gracious common dining areas, and many additional amenities such as housekeeping, planned activities, beauty salons and transportation services.

Skilled Nursing

When the need for the highest level of medical and nursing care presents itself, you have many choices to make from the 27 skilled nursing communities located throughout the links on our website. Through extensive rehabilitation and community re-entry programs the communities work daily to attain goals of returning residents to their highest level of independence.

There is a genuine sense of mission and purpose among the nursing staff in each community, which allows respect and compassion to shine through all they do. With a choice of a private or shared room, the skilled nursing communities offer around-the-clock nursing service.

Diverse services offered by the skilled nursing communities include: extensive rehabilitative services, such as physical, occupational, and speech therapy, restorative nursing care, medication administration, diabetic monitoring, extensive wound care, bowel and bladder training, are just a glimpse of services provided.
