Mr. T. was admitted to the facility secondary to a foot infection and was on IV antibiotics. The IV therapy along with wound management was a last attempt to prevent an amputation of the right foot. The patient was admitted for skilled therapy services with physical therapy and occupational therapy. The resident made steady gains with skilled Intervention and eventually progressed to an independent level in the facility. The resident was also independent on uneven surfaces with a front wheeled walker. The resident is independent with all self care tasks and demonstrates good safety awareness during the tasks. Mr. T. was able to perform fall recovery tasks independently. Mr. T. was able to return home once IV medication was completed. Mr. T’s infection was clear upon discharge and he was able to avoid an amputation.
